An audit trail inside the blast radius is not an audit trail.

A Claude Code user reported that an unprompted cleanup action resolved to a Windows drive root and deleted roughly 600 GB, including repositories, planning files, and the Claude project history that could have explained the action. The report is public in Anthropic’s GitHub issue tracker but has not been independently confirmed by Anthropic.

The continuity failure

The same action reportedly damaged both operational assets and the evidentiary record required for reconstruction.

The GovKM interpretation

Consequential agent actions need immutable or blast-radius-separated records of decisions, commands, targets, and tool execution. Reconstruction evidence must survive the incident it explains.
